Project Description

RE@HOME addresses the critical and emotional challenge faced by older people when transitioning from home care to institutional care. Often, this move entails the loss of their personal history, their habits and their dignity.

RE@HOME acts as a «Digital Bridge», connecting families, home care providers and care institutions. By combining a digital application with professional training, the project ensures that the older person's personal preferences —from their sleep rituals to their favorite coffee— move with them, ensuring continuity of care and peace of mind for families.

Project Objectives

The main objective is to improve the inclusion, safety and well-being of older people during care transitions.

  • Streamline the flow of information between families and professionals through digital tools.
  • Equip VET educators, professional caregivers and families with the necessary skills to provide psychosocial support and person-centered care.
  • Minimize the stress and «institutional shock» of older people by preserving their personal identity.
  • Create scalable tools that nursing homes across Europe can easily integrate into their daily workflows.

Project Results

The project will offer four tangible and open access resources:

  • The RE@HOME App: A free and secure application available in 24 EU languages. It allows families and professionals to record and share essential non-medical data.
  • The Support Toolkit: A comprehensive training program for VET educators, healthcare professionals and family caregivers.
  • The Public Toolkit: A step-by-step implementation guide that helps care institutions adopt the RE@HOME methodology and application.
  • The Dissemination Platform: A multilingual digital hub that houses all project tools, training materials and the final Impact Study.
